Changes to Evaluation and Management Services in 2021

In the 2019 MPFS final rule, CMS finalized changes to the coding and payment structure for Evaluation & Management (E&M) services effective calendar year (CY) 2021. In the 2020 proposed rule, CMS is proposing to reverse some of these finalized policies and align E&M coding and payment with changes adopted by the Current Procedural Terminology (CPT) Editorial Panel for E&M services. The changes include:

  1. Eliminating history and physical as element for code selection.
  2. Allow physicians to choose whether their documentation is based on medical decision making (MDM) or total time.
  3. Deletion of CPT code 99201 (new patient level 1 E/M service).
  4. Creation of a shorter prolonged services CPT code.
